DASH 2021 sign-up form is closed

The DASH sign-up form is now closed. The ability to sign up to use DASH closed at midday on Friday 19th November, in order to allow us time to process registrations and distribute bespoke survey links back to organisations ahead of the DASH survey itself closing at midnight on Sunday 21st November 2021.
